INCIDENT REPORT GENERATOR

Standardized after-action documentation for patrol operations. Fill out what happened. Get a clean, structured report you can save or print.

Privacy and Security Everything stays in your browser. Nothing is sent to any server. No data is stored after you close the page. Use operational callsigns, not legal names. Redact any information that could compromise someone's safety or identity.
Be specific. Intersections, landmarks, block numbers. Avoid exact addresses of private residences.
Use descriptors, not legal names. Example: "Male, approx 30s, grey hoodie, agitated". Include yourself and any team members present.
Be factual. Describe what you did and said, not what you felt or assumed. "I said X" not "I tried to calm them down".
Attach photos from the scene. Each photo can have a caption describing what it shows. Photos are embedded directly in the report. Faces of uninvolved persons should be obscured before uploading.
